Sea Breeze Family Practice has been serving the people of Carteret County since 1998.  We are a full service Family Practice that cares for adults and children in both an outpatient and inpatient setting.  We provide a continuity of care for patients from the office setting and into the hospital setting at Carteret General Hospital. 

We currently are staffed by a Board Certified Family Practitioner and two Board Certified Physician Extenders.  We all work very close together to provide quality care for your entire famliy.

NEW PATIENTS:

If you have an upcoming appointment in our office.  Please go to our "Links" page to print off our "New patient registration" form and our "Financial Policy" form.  Feel free to sign and fax this to our office or just bring them in with you to your appointment.

NARCOTIC PRESCRIPTIONS:

Please be advised that we have strict guidelines which we follow when treating patients with narcotic prescriptions.  If you are a new patient you must read and sign our Narcotic Prescribing Policy.  You must also sign a waiver that will allow us to research your name on the North Carolina Controlled Substance Reporting System.  On the "Links" page you must print and sign both the "Narcotic Permission" form and the "Narcotic Prescribing Policy" form.  Please fax this to the office or bring it with you to your office visit.  You will not be given any narcotic prescriptions on your first visit.  We do not write for methadone or large quantities of pain medications.
